ACE Foundation of Florida is a Jacksonville community nonprofit rooted in outreach and care coordination. We meet neighbors where they are, advocate alongside them, and connect them to housing, medical care, and the resources they need — then follow up so no one is left behind.

Getting help can be confusing and overwhelming. ACE Foundation works alongside you — listening first, then connecting you with health services, essential supplies, recovery support, and community partners, and coordinating the follow-up so no one falls through the cracks.
